[Bucardo-general] Additional log informaiton when an error happens...

I'm currently struggling to keep a 4.5.x service up and stable... I keep
getting disconnects, it would be really useful if the disconnect tells
you which server disconnected as this only tells which table:

closed the connection unexpectedly\n    This probably means the server

Not which server (and I have 9 servers)
