[check_postgres] a check for hitting autovacuum_max_freeze_age?

Good idea, Rob, this is a new action as of version 2.1.0 called 
"autovac_freeze".

What Rob and I ended up with after some discussion is just setting the
warning and critical to default to 90% and 95%, same as many of the other
actions.
